If your cholesterol is high, you have a family history of early heart disease, or you simply want to know your real cardiovascular risk and what to do about it, Dr Imtiaz Ali Kalyar can review it by video from anywhere in Ireland.
The Consultation
An online prevention review covers your full lipid profile and other bloods, your cardiovascular risk factors, your family history, and a formal 10-year risk estimate using the European SCORE2 tool. Where relevant it also screens for familial hypercholesterolaemia. You leave with your risk category, a personalised LDL-cholesterol target, a decision on whether lipid-lowering medication is warranted, and a specific lifestyle plan.
It is carried out personally by Dr Imtiaz Ali Kalyar, Consultant Interventional Cardiologist (FRCP London, IMC 102093, GMC 6082421), by video or phone, most weekday evenings between 16:00 and 19:00. The fee is the same as an in-person consultation: €230 initial, €150 follow-up.

Dr Kalyar uses SCORE2 (and SCORE2-OP for people aged 70 and over), the current European Society of Cardiology-recommended tool, which estimates your 10-year risk of a first fatal or non-fatal heart attack or stroke from your age, sex, smoking status, blood pressure and cholesterol. The result places you in a risk category, which in turn sets your LDL-cholesterol target and whether medication is recommended alongside lifestyle change. People with existing cardiovascular disease, diabetes with organ damage, or familial hypercholesterolaemia are automatically high or very-high risk and are managed accordingly.
Yes, where it is indicated. Dr Kalyar will explain the expected absolute benefit for your risk level, discuss side effects and the evidence around them, and recommend a specific agent and dose. If you have had muscle symptoms on a statin before, alternatives and strategies for statin intolerance — including lower doses, alternate-day dosing and ezetimibe — are discussed. The plan and a baseline blood-test request go to your GP, who issues the prescription.
It assesses your personal risk in that context, screens for familial hypercholesterolaemia using your LDL level, personal and family history, and where appropriate recommends genetic testing and cascade screening of first-degree relatives. Familial hypercholesterolaemia affects roughly 1 in 250 people and is substantially underdiagnosed; identifying it changes both your treatment targets and your family’s.
Yes. Online cardiology consultations are open to adults anywhere in the Republic of Ireland — Dublin, Cork, Galway, Limerick, the midlands, the west and rural areas included. You do not need to travel to Naas for the consultation itself. If a hands-on examination or a test that must be done here is needed, that is arranged separately at the Naas clinic on a Saturday or Sunday.
No. Both self-referral and GP referral are accepted for online cardiology consultations. A GP referral letter is helpful because it provides your history, medication list and recent results, and can be sent securely to the clinic through Healthmail, but it is not required to book.
Yes. A clinic letter summarising the assessment, findings and plan is sent to your GP after every online consultation, the same as after an in-person visit, so your primary care team can continue shared care and issue or adjust prescriptions locally where appropriate.
The same as an in-person visit: €230 for an initial consultation and €150 for a follow-up. This is a self-pay clinic. Patients with private health insurance may be able to claim outpatient consultant benefits depending on their policy, and should confirm eligibility directly with their insurer before booking.
An online consultation is not for emergencies. Chest pain or tightness, pain spreading to the arm or jaw, sudden severe breathlessness, fainting or a blackout, or a fast irregular heartbeat with chest pain or collapse all need immediate attention — call 999 or 112 or attend your nearest Emergency Department. This clinic does not provide emergency or same-day acute care.
