Palpitations are common and are usually not dangerous — but they need a proper history and, often, a look at a rhythm recording to be sure. If you have had palpitations, been told you have atrial fibrillation, or had an abnormal ECG or smartwatch alert, Dr Imtiaz Ali Kalyar can review it by video from anywhere in Ireland.
The Consultation
An online palpitations or AF review covers a detailed symptom history, interpretation of any ECGs, Holter or event-monitor recordings, smartwatch or single-lead traces you have captured, your stroke-risk profile if you have atrial fibrillation (CHA₂DS₂-VASc), and your current medication. You leave with an explanation of what is likely going on, a monitoring or investigation plan, and a treatment plan where one is needed.
It is carried out personally by Dr Imtiaz Ali Kalyar, Consultant Interventional Cardiologist (FRCP London, IMC 102093, GMC 6082421), by video or phone, most weekday evenings between 16:00 and 19:00. The fee is the same as an in-person consultation: €230 initial, €150 follow-up.

Stable atrial fibrillation is well suited to a video review. Dr Kalyar can interpret your ECG and monitor recordings, calculate your stroke risk with the CHA₂DS₂-VASc score, discuss whether anticoagulation is indicated, and review rate- or rhythm-control medication. What has to happen in person or locally is the rhythm monitoring itself, a resting ECG, and an echocardiogram. New AF with a fast heart rate and symptoms such as chest pain, breathlessness at rest or feeling faint needs emergency assessment, not a booked consultation.
A smartwatch or single-lead trace is a useful prompt but not a formal diagnosis. It is helpful to bring the actual traces (exported as PDF) to the consultation. Dr Kalyar will review them and, in most cases, arrange a medical-grade recording — a 12-lead ECG and a period of Holter or event monitoring — to confirm or exclude an arrhythmia before any treatment decision.
Anything you have: previous ECGs, Holter or event-monitor reports, and exported smartwatch or single-lead PDFs, ideally labelled with the date and whether you had symptoms at the time. A symptom diary — when episodes happen, how long they last, what you were doing — is also valuable.
Yes. Online cardiology consultations are open to adults anywhere in the Republic of Ireland — Dublin, Cork, Galway, Limerick, the midlands, the west and rural areas included. You do not need to travel to Naas for the consultation itself. If a hands-on examination or a test that must be done here is needed, that is arranged separately at the Naas clinic on a Saturday or Sunday.
No. Both self-referral and GP referral are accepted for online cardiology consultations. A GP referral letter is helpful because it provides your history, medication list and recent results, and can be sent securely to the clinic through Healthmail, but it is not required to book.
Yes. A clinic letter summarising the assessment, findings and plan is sent to your GP after every online consultation, the same as after an in-person visit, so your primary care team can continue shared care and issue or adjust prescriptions locally where appropriate.
The same as an in-person visit: €230 for an initial consultation and €150 for a follow-up. This is a self-pay clinic. Patients with private health insurance may be able to claim outpatient consultant benefits depending on their policy, and should confirm eligibility directly with their insurer before booking.
An online consultation is not for emergencies. Chest pain or tightness, pain spreading to the arm or jaw, sudden severe breathlessness, fainting or a blackout, or a fast irregular heartbeat with chest pain or collapse all need immediate attention — call 999 or 112 or attend your nearest Emergency Department. This clinic does not provide emergency or same-day acute care.
