Most Type 2 diabetes is well managed in primary care. A consultant diabetologist is valuable when HbA1c stays above target despite treatment, when hypoglycaemia is a problem, for Type 1 diabetes and insulin-treated Type 2, for starting or troubleshooting insulin pumps and continuous glucose monitors, for GLP-1 and newer therapies, for diabetes in pregnancy planning, and for interpreting a confusing picture such as suspected LADA or MODY. Dr Kazmi works with your GP and sends a clinic letter after every consultation.
Yes — this is one of the most effective uses of a video diabetes consultation. If you share your LibreView or Dexcom Clarity report before the appointment, Dr Kazmi can review your time-in-range, glucose variability, overnight patterns and hypoglycaemia frequency and use that to adjust insulin or other medication precisely. Two weeks of recent data makes the consultation far more productive than readings recalled from memory.
For most people, yes. Insulin titration is based on glucose patterns, HbA1c, hypoglycaemia history, diet and activity, all of which can be reviewed in detail remotely. Dr Kazmi sets out specific dose changes and a plan to check their effect, communicated to your GP. Where control is very unstable, where there is severe hypoglycaemia with impaired awareness, or where a new regimen is being started from scratch, closer follow-up or an in-person review may be advised.
These parts of diabetes care need to be done in person or locally: the diabetic foot examination and neuropathy testing, retinal screening through the national programme or a local optometry service, and blood and urine tests for kidney function. Dr Kazmi coordinates these — identifying what is due, arranging it locally to you, and reviewing the results — so the specialist appointment focuses on the decisions that follow.
Dr Kazmi assesses suitability for continuous glucose monitoring and insulin pump therapy and advises on the options and on funding routes in Ireland, including the HSE Long Term Illness scheme and manufacturer pathways. Yes. Consultations are available by secure video or phone to adults anywhere in the Republic of Ireland, most weekday evenings between 16:00 and 19:00. In-person appointments run in Naas, Co. Kildare on Saturdays and Sundays. Where a physical examination or an on-site test is needed, the in-person visit is arranged as a follow-up to the video consultation rather than instead of it.
No. Both self-referral and GP referral are accepted for all consultations. A GP referral letter is useful because it provides your history, medication list and recent results, and can be sent securely to the clinic through Healthmail, but it is not required to book.
Yes. A clinic letter setting out the assessment, findings and plan is sent to your GP after every consultation, in person or online, so your primary care team can continue shared care and issue or adjust prescriptions locally where appropriate.
