High blood pressure is one of the most manageable cardiovascular risk factors — and one of the most suited to a remote consultant review. If your readings are high, your current tablets are not controlling things, or you have never had a proper specialist assessment, Dr Imtiaz Ali Kalyar can review it by video from anywhere in Ireland.

What This Online Review Covers

An online blood pressure review looks at your home and clinic readings, any ambulatory (24-hour) monitoring you have had, your bloods and kidney function, other cardiovascular risk factors, and your current medication. The output is a clear diagnosis, a medication plan, a monitoring plan, and targets tailored to you, with a letter to your GP so prescriptions can be issued or changed locally.

It is carried out personally by Dr Imtiaz Ali Kalyar, Consultant Interventional Cardiologist (FRCP London, IMC 102093, GMC 6082421), by video or phone, most weekday evenings between 16:00 and 19:00. The fee is the same as an in-person consultation: €230 initial, €150 follow-up.

Can high blood pressure really be managed by video?

For most people, yes. Blood-pressure management is driven by readings, blood results and risk factors rather than by a hands-on examination. Dr Kalyar reviews your home and any 24-hour monitoring data, your bloods and kidney function, and your medication, then sets a diagnosis, targets and a treatment plan. An in-person examination or an ambulatory monitor fitting is arranged where it will change management. Resistant or unusually severe hypertension, or a suspected secondary cause, may need earlier in-person assessment.

What readings should I bring to the consultation?

Ideally a week of home readings: twice in the morning and twice in the evening, taken sitting after five minutes’ rest, with a validated upper-arm monitor. Bring the average, not just the highest reading, plus any 24-hour ambulatory report and recent blood tests. The more data you send in advance, the more the consultation can focus on the plan.

Can medication be changed after an online blood pressure review?

Yes. Dr Kalyar can recommend starting a new agent, increasing a dose, combining tablets or switching because of side effects. The recommendation and rationale are set out in a letter to your GP, who issues the prescription. Where a blood test is needed first — for example before or after starting certain tablets — that is arranged locally and reviewed at a short follow-up.

Can patients outside Co. Kildare use this service?

Yes. Online cardiology consultations are open to adults anywhere in the Republic of Ireland — Dublin, Cork, Galway, Limerick, the midlands, the west and rural areas included. You do not need to travel to Naas for the consultation itself. If a hands-on examination or a test that must be done here is needed, that is arranged separately at the Naas clinic on a Saturday or Sunday.

Do I need a GP referral to book online?

No. Both self-referral and GP referral are accepted for online cardiology consultations. A GP referral letter is helpful because it provides your history, medication list and recent results, and can be sent securely to the clinic through Healthmail, but it is not required to book.

Will my GP receive a letter after the consultation?

Yes. A clinic letter summarising the assessment, findings and plan is sent to your GP after every online consultation, the same as after an in-person visit, so your primary care team can continue shared care and issue or adjust prescriptions locally where appropriate.

What does an online cardiology consultation cost?

The same as an in-person visit: €230 for an initial consultation and €150 for a follow-up. This is a self-pay clinic. Patients with private health insurance may be able to claim outpatient consultant benefits depending on their policy, and should confirm eligibility directly with their insurer before booking.

What symptoms need emergency care rather than an online consultation?

An online consultation is not for emergencies. Chest pain or tightness, pain spreading to the arm or jaw, sudden severe breathlessness, fainting or a blackout, or a fast irregular heartbeat with chest pain or collapse all need immediate attention — call 999 or 112 or attend your nearest Emergency Department. This clinic does not provide emergency or same-day acute care.
